If a student has been attending another school and transfers to your school, the student must request the change. You must also report the change to VA.
You must get a signed and dated request for the change from the student. The student can complete a hard copy of VA Form 22-1995 or 22-5495, complete a form you create for change requests, or sign and date the certification you keep in the student's file.
The certification you keep in the student's file will be a result of you entering the change in VA-ONCE. You will learn the steps to complete this process in the VA-ONCE lesson.
