DEA-Chapter 35 students use VA Form 22-5495, Request for Change of Program or Place of Training (Survivors' and Dependents' Educational Assistance) to identify and request approval for a change when transferring schools.

For example, Sam is currently enrolled in Conus University and is receiving DEA-Chapter 35 VA education benefits; however, he will be moving out of state and wants to enroll in another college. Sam wants to ensure that he will maintain his VA education benefits when he moves to the new school. VA needs to be notified of Sam's change of place of training.

Sam can submit VA Form 22-5495 electronically through VONAPP or should provide you with the completed form.
